PROCEDURES FOR FILING NON-LEAGUE GRIEVANCES
 
Any written grievance shall be sent by first-class, certified, or registered mail to the Grievance Chairperson, c/o USTA   Executive Director, 70 West Red Oak Lane, White Plains, New York 10604, within 30 days following the date on   which the alleged violation occurred; provided, however, if the alleged violation is based on a falsification of age or other information, the written grievance shall be mailed within 30 days after the Tournament Chairperson has been advised of such falsification. Absent good cause shown, grievances not mailed within 30 days are barred.
