Before the date of the workshop:
At least 1 Month in Advance:
_____ Submit an application for approval
Immediately after your application is approved:
_____ Promote the Workshop (20 participants needed)
_____ Register participants & collect fees
2 weeks in advance:
_____ Confirm estimated participant number
_____ Contact your trainer, discuss details
1 week in advance:
_____ Review workshop details/schedule with trainer
_____ Make plans for lunch and beverages
2-3 days in advance:
_____ Receive and unpack materials provided by the USTA through NetKnacks
_____ Prepare participant packets from received materials
_____ Fill in Workshop Code on evaluation forms
The day of the workshop:
_____ Arrive 1 ½ -hours early for set-up
_____ Assist the trainer with set-up
_____ Greet and register the participants
_____ Distribute participant manual and Learn to Rally and Play Book at registration
_____ Distribute lunch
_____ Collect participant evaluations & distribute remaining participant materials as
evaluations are turned in
_____ Complete Host Site Evaluation
_____ Review participant evaluations with the trainer
After the workshop:
_____ Complete W-9 form for reimbursement
_____ Return Host Site Evaluation, W-9 and participant evaluations to the USTA National
Office
_____ Return extra sets of materials to the USTA National Office
