2009 Data Cards are due to Sectional Chairs no later than October 15, 2009.
Data Card Procedures
The following are the instructions for your Data Card
- The Chief of Umpires or Referee must sign the Data Card at the events that you officiate.
- You must fill in the school that you attended in 2009. The space for this information is located on the inside of the card, upper right, above the work record.
- When indicating an event that you officiated, please include the date of the event.
- Chair Umpires should indicate the number of matches officiated on the work record. Referees, Roving officials and all Line Umpires should indicate the number of days officiated on the work record.
- When you receive your Data Card, fill out the information on the front of the card. You are required to fill-in the following information: Name, USTA#, Most Current Expiration Date and Signature. If you have had any changes to the other information during the year, enter and highlight those changes and check the appropriate box on the Data Card to indicate a change. The back of the card is to be filled in ONLY by the Sectional Chairman.
- The Data Cards are due to your Sectional Chairman by October 15, 2009. (Note: Some large sections may set an earlier date.) If you plan to work events after the deadline, indicate those events on the card. DO NOT send your data card to the National Office. DO NOT DELAY sending in the card to your Sectional Chairman, who has a deadline soon after to send the data cards to the National Office. Any official who fails to get their data card to their sectional chair by October 15th risks de-certification with possible demotion to Provisional Umpire status.
- Only original Data Cards will be accepted by the National Office (no photo copies will be accepted).
- Make a copy of your Data Card before sending it to your Sectional Chairman. If it is lost and you do not have a copy, you will have to reconstruct the entire work record.
- All Officials are required to fill out his/her USTA Officials Data Card in full. Incomplete data cards will not be accepted by the USTA Officials Department.
- Please note that there is "no vision requirement" for all certified officials in 2008. All certified officials are required to get an eye exam during 2009 in order to be considered for a 2010 USTA certification.
Provisional Officials who have met the requirements may move to a sectional certification level within the same year. (All other officials must wait until the following year to move up to a higher certification level.)
