USTA Northern California is conducting a tennis census to learn how we can improve tennis within our section. Whether you are a seasoned player, coach or provider, just getting started or have never even picked up a racquet before, we want to learn more about your experience with tennis. Your insights are a critical part of helping us understand the different pathways of tennis participation and will help fuel the growth of our sport.

 

We would appreciate you taking this anonymous census. It should only take about 10 minutes to complete, and there are no right or wrong answers—your honest input is most valuable. Plus, enter the chance to win a $25 Tennis Warehouse gift card upon completion of the census! More details are below.

 

Please note, you must be 13 years and older to participate. Your privacy is important and all responses will be kept confidential. No individual information will be identifiable.

Complete the Census: Enter to Win a $25 Gift Card

Upon completion of the census, find details on the last page to enter the chance to win a $25 Tennis Warehouse gift card. We will randomly select five winners within a few weeks of the close of the census when we hit 1,000 submissions. Terms and conditions apply. Please review below.

Terms & Conditions:

  1. To enter into the Tennis Warehouse Gift Card Raffle, each entrant must complete the USTA Northern California Tennis Census and show proof of completion if asked and submit their name and required information through the provided Google Submission Form.
  2. Entry to the Tennis Warehouse Gift Card Raffle is available only to persons over the age of 13. You may be asked, at any stage, to provide USTA Northern California with proof of age and /or identity.
  3. Employees and relatives of employees of USTA Northern California shall not be eligible to enter the Raffle. For the purposes herein, the term "relative" shall mean spouse, partner, parent, child or sibling.
  4. Entry into the Tennis Warehouse Gift Card Raffle will stay open until the close of the USTA Northern California Tennis Census when 1,000 submissions have been acquired. At the end of the USTA Northern California Tennis Census, five winners will be selected to win a $25 Tennis Warehouse gift card. USTA Northern California will conduct a random drawing from among all entries received by the close of the USTA Northern California Tennis Census when 1,000 submissions have been acquired. The winners will be notified by email. Non-winning entries in any drawing will not be rolled over and included in any subsequent drawings.
  5. USTA Northern California shall notify winners of the winning via email within five business days of the close of the USTA Northern California Tennis Census. The winner must accept the prize by email or in person within two weeks of notification by USTA Northern California of winning. USTA Northern California is not responsible for any delay or failure to receive notification for any reason, including inactive email account(s), technical difficulties associated therewith, or Winner’s failure to adequately monitor any email account.
  6. Each entrant shall be entitled to make a single entry upon proof of completion of the USTA Northern California Tennis Census. Entry into the Raffle is free. In the event that USTA Northern California deems an entrant to have in any way acted in bad faith with respect to the Raffle, such entrant shall be excluded from the Raffle and consequently deemed ineligible to win the Prize, as defined in these terms and conditions.
  7. In order to enter the Raffle each entrant shall submit their first name, last name and email address. This information will remain confidential and will not be used for anything outside of the capacity of the Raffle.
  8. The Prize for winning the Raffle shall be the offer of one gift card to Tennis Warehouse with the value of $25 to five winners. There will be five total winners.
  9. Winners specifically acknowledge that winners shall be responsible for the payment of all fees outside of those offered as part of the package and airfare, taxes, title fees, license fees, insurance requirements and other charges as may be required under any law, ordinance, statute, regulation or otherwise.
  10.  USTA Northern California’s decision is final with respect to all matters relating to awarding of the Prize and shall not be subject to review or appeal by any entrant or by any third party.
  11. The Prize is neither transferable nor exchangeable and cannot be exchanged for money or money’s worth.
  12. By entering the Raffle, each entrant unreservedly agrees to these terms and conditions which govern the Raffle and the awarding of the Prize.
  13. By entering the Raffle each entrant agrees to release, discharge and hold harmless the USTA Northern California its legal representatives, affiliates, subsidiaries, agencies and their respective officers, directors, employees and agents from any damages whatsoever suffered, sustained or allegedly sustained in connection the Raffle or the acceptance of the Prize.
  14. The winning entrants shall be solely responsible for any taxes levied in relation to the delivery or receipt of the Prize.
  15. USTA Northern California reserves the right to alter these terms and conditions at any time and in its sole discretion.
  16. USTA Northern California reserves the right, at any time, to cancel, modify or suspend the Raffle if, in its sole judgment, the Raffle is not capable of being conducted as specified.
  17. These terms and conditions and any matters relating hereto shall be governed by and construed in accordance with the laws of California and jurisdiction over any and all disputes shall be exclusive to the state and federal courts in Northern California.
  18. USTA Northern California, its affiliates, subsidiaries, agencies, and representatives and their respective directors, officers, shareholders, and employees are not responsible and shall not be liable for: (i) electronic, hardware or software program, network, internet or computer malfunctions, failures, or difficulties of any kind, including without limitation, server malfunction or by any error (human or otherwise) which may occur in the processing of entries; (ii) failed, incomplete, garbled or delayed computer transmissions; (iii) late, lost, misdirected, incomplete entries; or (iv) any condition caused by events beyond the control of USTA Northern California that may cause the promotion to be disrupted or corrupted. If, for any reason, an entry is confirmed to have been erroneously deleted, lost or otherwise destroyed, corrupted or for any other reason not accepted as an entry in the Raffle, entrant’s sole remedy is to enter the Raffle again and to receive another entry. USTA Northern California reserves the right in its sole discretion to cancel or suspend the sweepstakes or any portion thereof should virus, bugs, or other causes beyond control of USTA Northern California corrupt the administration, security, or proper play of the Raffle. USTA Northern California further reserves the right to modify these Terms & Conditions or modify, suspend, or terminate the Raffle at any time in its discretion.
