RETURN TO PLAY WAIVER - Adult | Junior
Health and safety of our players is a top priority at all times, but even more so during this pandemic.
There are steps that we all must and will take to encourage a safe return to tennis. One important step for players is to make sure they understand the risks involved in USTA Southwest program participation in this time.
We require you to review and sign the attached USTA Southwest waivers prior to participation.
