Medical waivers are given on a HIGHLY selective basis. Decisions on waivers will be made by the USTA Southern Junior Tennis Waiver group by a majority vote. Requests will be submitted through the form listed above and must be submitted at least one week prior to the tournament entry deadline. Waiver considerations include, but are not limited to, illness and injury.
